Why patients miss appointments, and which reasons you can actually do something about

Most no-show advice treats it as one problem. It is at least four, and only three of them are yours to fix. Knowing which is which stops you spending money on the wrong one.

Ask a clinic owner why patients miss appointments and you will usually hear one answer: they forget. It is the most comfortable answer, because it has an obvious fix that somebody else can implement. Send more reminders. Buy the reminder software. Done.

In practice a missed appointment is at least four different problems wearing the same coat, and they need different things. If you treat them as one, you end up sending more messages to people who were never going to come, and none to the ones you could have kept.

They genuinely forgot

This is the one everybody plans for, and it is real, particularly for appointments booked more than a fortnight out. It is also the easiest to solve and the cheapest. A reminder at the right distance from the appointment, on a channel the patient actually reads, closes most of it.

The mistake is sending it too early. A reminder seven days out gets read and then forgotten again. One the afternoon before is worth more than three the week before, and it costs less.

Something changed and telling you was awkward

This is the biggest one in most clinics and almost nobody measures it. The patient knows by Tuesday that they cannot make Thursday. Cancelling means ringing during opening hours, waiting, and then explaining themselves to a person. Not turning up costs them nothing and involves no conversation.

You cannot remind your way out of this. You have to make changing an appointment easier than not showing up. One tap, no phone call, no explanation required. Clinics are often reluctant here, because making cancellation easy feels like inviting it. The opposite happens: you find out on Tuesday instead of Thursday, and Thursday's slot is still sellable.

They were never really booked

Somebody rang, a time was written down, and no confirmation was ever exchanged. From the clinic's side that is a booking. From the patient's side it was a conversation about a possible booking. The gap between those two positions is invisible until the slot is empty.

A confirmation step turns a soft booking into a real one, and just as usefully, it tells you which of your bookings are soft while there is still time to do something.

They were not coming back anyway

Some missed appointments are the visible end of a decision the patient made a while ago. They were unhappy, or they got better, or they went somewhere else. Reminding them harder will not change it, and it is the one category where extra contact does damage.

This category is small in most clinics, but it matters because it is the reason blanket reminder campaigns produce worse results than targeted ones.

How to find out which you have

You do not need software to do this, and you should not buy any until you have. Take last month's missed appointments, however you record them, and put each one in one of the four buckets. If you cannot tell which bucket, that is a finding too: it means nobody asked afterwards.

  1. 01Count themPull every missed appointment from the last full month. An approximate list is fine; the shape matters more than the precision.
  2. 02Bucket themForgot, could not tell you, never really booked, not coming back. Guess where you have to, and mark the guesses.
  3. 03Look at the biggest bucketThat is where your money is. In most clinics it is the second one, which is also the one reminder software does nothing about.
  4. 04Fix one thingWhatever the biggest bucket needs. Then measure the same month next year, not next week.
If more than half your missed appointments are patients who could not tell you, buying a reminder system will disappoint you.

None of this needs us. If you do the exercise and the answer is that your reminders are already fine and your cancellation path is already one tap, then your attendance problem is small and you should spend the money elsewhere. We would rather tell you that than sell you a project.
